Specialised treatments for fertility problems

In our clinic in Las Palmas de Gran Canaria we offer our patients state-of-the-art treatments to promote pregnancy in case of male or female fertility problems or both.

When is treatment indicated?

Treatment is prescribed when the alterations arising from the above-mentioned studies need to be corrected and will depend on the pathology diagnosed. It may be conservative or surgical treatment. In the particular case that the patient has no spermatozoa in the semen analysis, a testicular biopsy (TESE) is recommended. This has a dual purpose:

  • Study the testicular tissue to find out what has happened and make a diagnosis.
  • Take advantage of the fact that we have a window into the testicle to try to obtain testicular spermatozoa in order to keep them frozen (cryopreservation), and then be able to use them in assisted reproduction treatment.

What happens if no sperm are obtained in the testicular biopsy?

The probability of finding spermatozoa in a testicular biopsy (TESE) is approximately 50%. In the event of not being able to obtain sperm, depending on the patient, we could opt for what is called the "microTESE", a testicular biopsy under microsurgery, in which we identify the healthiest testicular structures to try to recover sperm from there with a probability of 20-30%.

What are the possible assisted reproduction treatments available to the couple?

It depends on each case, in our clinic located in Las Palmas de Gran Canaria we establish a protocol adapted to the patient's problem: we can choose to continue the spontaneous search for pregnancy or to recommend assisted reproduction treatment.

If there is a seminal infection, treatment will be with antibiotics. In case of varicocele, varicocele surgery is recommended. In those patients in whom sterility is caused by a previous vasectomy, a TESE or reversal of the previous vasectomy with microsurgery of the seminal duct will be prescribed. In the case of uterine or ovulatory disorders, they will be dealt with on an ad hoc basis. If we are faced with an alteration that does not allow pregnancy to be achieved, it is recommended that an Assisted Reproduction Technique be performed:

  • Artificial Insemination
  • Intraoocyte Sperm Microinjection (ICSI)

How do we find out if we are pregnant?

In patients who spontaneously seek pregnancy, after one or two weeks of menstrual delay, a pregnancy test can be performed. Patients who have undergone assisted reproduction treatment can take a pregnancy test 2 weeks after treatment.

Can I treat during the COVID-19 pandemic?

Yes, if the person seeking treatment suspects COVID-19 infection, it is recommended to postpone seeking pregnancy until the patient's condition is resolved. In case of vaccination, although data are limited, the European Medicines Association (EMA) certifies that animal studies have shown no negative effects on pregnancy. As a precaution, it is recommended to wait at least two weeks after receiving the second dose of vaccine before starting treatment.
